Respiration extends how long you can stay underwater before you start drowning — and also cuts the odds of losing air each tick you're submerged.
What Respiration actually does
Each level adds roughly 16 seconds to how long your air supply lasts before you start taking drowning damage: about 32 seconds total at level I, 48 at level II, 64 at level III (vs. 16 seconds with no Respiration at all). Separately, it also reduces the chance your air supply ticks down at all on a given tick — 50% at level I, ~67% at level II, 75% at level III — meaning both effects combine to make underwater time last considerably longer.
How to use it
Respiration is for anyone spending real time underwater — ocean exploration, monument raids, underwater builds — without constantly surfacing for air. It has no interaction with Aqua Affinity (which speeds up mining underwater instead of extending breath) — many players run both on the same helmet.
Well-known builds that use it
Respiration III is part of The Ideal Helmet Enchantments, alongside Protection, Aqua Affinity, Thorns, Unbreaking, and Mending.

Frequently asked questions
How much extra time underwater does Respiration III give?
Around 64 seconds total before drowning damage starts, versus 16 seconds with no Respiration — plus a separate 75% chance each tick that your air supply doesn't tick down at all.
Does Respiration help with mining speed underwater?
No — that's Aqua Affinity's job. Respiration only extends how long you can stay submerged before drowning; many players run both enchantments on the same helmet since they don't conflict.
